I don't remember title or author, but the plot of it was after the war, Harry wanted to do something meaningful, and ended up becoming guardian to 6 or 8 children of various ages that were orphaned by the war. He bought/inherited a piece of property called Heathmoor House, but the only person who knew where he was was Severus. During winter break one of the oldest children flooed back to Hogwarts in a panic because Harry was very ill. It goes from there, and it ends with them getting married.
