Shirou is possessed by Angra Mainyu after the fire. But only a little bit. Enough so that he might think about shoving his thumbs in Issei's eye sockets and popping his eyes out while talking to him about stuff that needs fixed around the school, but not enough that he would actually do it. But when it's something that he himself actually wants to do, Shirou has some trouble resisting. As in, he mostly can't.

It started one day several years before the Fifth War, when Taiga was being innocently fanservice-y. But as she teased him, he couldn't take it, pinned her down and raped her. Taiga didn't really do anything after because A)she provoked it and could have stopped him at any point (kendo prodigy, remember) and B)she's already kind of planning on having him take care of her for the rest of her life at this point, might as well try to keep those other bad girls from getting their claws in him.

Then when he gets angry at Shinji for hitting Sakura, he winds up killing him (I favor shoving him into traffic). Good for Sakura in that one of her abusers is gone, bad in that the worms still hunger. Shirou seems like the most suitable choice, from Zouken's perspective, and so he takes control of Sakura's body one evening and seduces Shirou. Sakura wakes up to find her senpai brutally fucking the shit out of her. As in, Shinji was practically gentle by comparison. But because of shenanigans with the Grail fragments in her and AM inside him, she can't help being incredibly aroused, no matter what he does. Just being around him is enough to give her a small orgasm. It also kick starts a bit of the healing she had in HF. Not the full "survive GoB" thing, but enough that no damage Shirou does lasts more than a few days. She was seeing clearly a couple hours after he decided he wanted to cum directly in her eye. When all is said and done, she's personally addicted to being around Shirou and being fucked by him, to the point of accepting anything he says or does. Kind of the Harley Quinn to Shirou's Joker.

Their situation gets noticed a little bit, leading Sakura to do a bit of work to make sure Ayako and the track trio will never oppose Shirou. Like, Sakura and Ayako are at archery later than everyone else, Ayako notices some bruises and/or that Sakura has been walking around with sex toys inside her all day, and tries to go off to do something about it. Next thing she knows, she's bound and gagged, her archery uniform pulled open and down, unspeakably aroused (which she would later find out was a bunch of aphrodisiacs), and Sakura is molesting her. Then Shirou comes in and Sakura happily tells him what's going on before facilitating the rape. By the end, Ayako is half-conscious, begging her new master to impregnate her. That sort of scenario.

The Grail War starts up, but because of the last two cluster-fucks and the Einzberns' hacking, the Grail decides to send out a Servant Ruler. Because AM is in it, it winds up incarnating Shirou as Servant Ruler, with all inherent abilities, including a power over other Servants. Thanks to that, he is able to pick up Caster and claim her when she collapses. Sakura still summons Rider at Zouken's command, and Shirou prepares to summon Saber.

Once he does, he finds that he has much the same desires as Gilgamesh. He wants to corrupt her, to see her break down from the perfection she clings to. Near constant torment, with Shirou readily fucking others in front of her, molesting her and then stopping at the point of orgasm, and ordering that she not masturbate or do anything to achieve release without his specific permission. When she finally begs for it, then he'll go to town on her, and leave her a drooling, whimpering pet, begging for more attention from her Master. Whether Saber Alter should show up is to your judgement.

Berserker and Archer go down fairly simply, since the Ruler isn't actually as impartial as the founders wanted. Illya is forced to watch Shirou rape her maids before he gets to her. Sakura goes to town on Rin, humiliating her in whatever way she can think of: foot-licking, rimjobs, piss-drinking, anything. By the time she's done, Tokiomi got his wish, kind of: his daughers had their final battle, and one emerged as the supreme victor. He just didn't expect exactly that outcome.

Bazett is found half-dead, and, similar to how Avenger wanted to murder her and rape her corpse, Shirou finds that being in a coma doesn't affect his desires. Same thing when Rin and Sakura's mother is brought home.

As for antagonists, Kirei can have his heart shut down by AM at any time, so he's just sort of waiting to see if they do that and debating the merits of selling his daughter. The heart-worm can get pulled out after Sakura gets Avalon'd, and Zouken gets owned. Gilgamesh goes down, either dying because he's opposing Ruler, or is genderswapped and winds up the lowest of the low within the group.

TL;DR, Shirou gets Angra Mainyu as an evil conscience and it turns him into a rapist that wins the Grail War with rape.

You know an interesting idea is what if in addition to sorta possessing Shirou, Angra Mainyu also affected and changed Shirou's Origin to something that isn't Sword. In the fanfiction Tainted Ideals Shirou's Origin is changed to "Flesh" due to Angra Mainyu partially affecting him early on. What his Origin there allows him to do is shape and affect organic matter, like wood or flesh easily. So Shirou there does stuff like turn his own bones into claws that are poisonous and such and can shape his own flesh and such. In fact, and the author confirmed this, Shirou there can make someone orgasm by touching them and playing with their nervous system with his magic.

Also you mention Ruler, well what if the Grail does send out Ruler, just a darker version here who helps Shirou in his raping?

Going with the "Flesh" idea, it could probably go all kinds of ways, like inverting pain and pleasure until they don't know what's what and it's all the same. Plus probable and myriad body modifications: altering breast sizes, making them futas, everything.

And Jeanne could obviously be used. Something like, she still gets incarnated in a human body and weakened because of interference from Angra Mainyu trying to keep her from interfering. Then when she meets Shirou, she recognizes the corruption and wants to try to save him, but because of said corruption, he's immune to all of her non-violent abilities. Trying to deal with him without hurting him gave enough time to touch her and use his "Flesh" to keep her from fighting back at all. After that, a project: even when she died, Jeanne d'Arc did not abandon her worship of God, praying while she burned at the stake. Shirou's goal is to have her renounce the god she prayed to and worship only him.

Nice. I can imagine Evil!Shirou here corrupting her to worship him instead. Ooh that would be the worst thing to screw over Bluebeard from F/Z if he was ever summoned for the 5th Grail War: his beloved Jeanne corrupted and definitely NOT a virgin after Evil!Shirou is through with her.
