I think it was a spell cast by a woman that got the spell wrong, something about being cast only by lovers, and John get swapped with either Sam or Dean. 'Sam/Dean' have to have physical relations in order to break the spell and return John to his body, even though he's technically with his son . . . meanwhile 'John' with Sam or Dean in him watches.
