She knows about Itachi's secret mission and knows that he is sick and will die in a few years, so Itachi made her to promise him that she will help Sasuke take revenge and restore the Uchiha clan. Because of the promise Naruko becomes Team 7 teacher, so when she trains her team she sends her brother Naruto and Sakura with a Shadow Clone while training Sasuke herself and in the end she lets him fuck her because she is planning to be his wife and the future and give birth to the new Uchiha clan. Sasuke on the other hands knows that Itachi loves Naruko, so he doesn’t mind fucking her as revenge on Itachi. Quote
