I remember it was on the author's livejournal page and that she had also written several Fall Out Boy fanfic. Here's what I remember: IT starts in the Primeval world with the team before an anomaly. Connor, Stephen, and another person go through it and end up stuck in the Torchwood world. They are found by Ianto and Tosh the remaining Torchwood three members along with another women who they had rescued. The world at that point has been overtaken by the tolcafane (sp?) and they end up having to run from them and hide in the hub. Connor ends up becoming a sniper and they rescue someone from a prison camp that has been set up. Eventually an anomaly appears and they run through it. They end up on a beach with an anomaly on the other side and having to race to get to it. When the go through they end up at the feet of the team they had left behind some time ago and find that no time had passed at all on this side.
